We emphasize both a social model and a medical model, and we believe that companionship is the key antidote for loneliness and boredom. We also believe that being of use, and being able to give as well as receive care makes being in need an easier burden to bear.
Big Meadows is located in the Northwest Illinois community of Savanna, a historical town on the banks of the Mississippi River. Nestled among rolling hills and beautiful trees. Deer, birds and other wild life can be observed. Residents are able to gaze out upon the flowering gardens in our scenic courtyard, or enjoy the beautiful aviary in our lobby.
Mission
Our goal is to assist each resident in reaching their maximum level of independence. We strive to provide the highest quality care and all the comprehensive rehabilitation services each individual resident requires to attain their highest functional abilities.
Care Plans
Big Meadows utilizes an interdisciplinary team approach in the evaluation, diagnosis and treatment of each resident. Under the direction of our Medical Director and local physicians, individualized care plans are developed to assist each resident in attaining their fullest achievable potential and to ensure that a program of the most comprehensive health care services is being delivered. The treatment team is comprised of professionals from a variety of clinical disciplines.
Restorative Nursing Care
Nursing care is provided 24 hours per day by our team of licensed nurses and certified nursing assistants. Nursing responsibilities include medication and treatment administration, training in self care skills and activities of daily living, providing range of motion exercises, taking care of personal care needs, and pressure sore treatment and prevention.
Connections Special Care Unit
We use the best parts of established programs for the care of people with Alzheimer's disease and other dementia illnesses. We know that we can make personal and relevant connections with the people we serve.

- Connections Unit staff consists of a Unit Director, an Activity Director, a Licensed Nurse and Certified Nursing Assistants who receive expanded specialized training in dementia care.
Connections is recognized as an Alzheimer's Special Care Unit by the Illinois Department of Public Health.
Rehabilitation & Therapy Services
Physical Therapy services are provided on-site in our Physical Therapy Clinic, under the direction of a licensed physical therapist. Occupational Therapy and Speech/Language Pathology are also available.
Activities
The Activity department utilizes recreation services to improve or maintain residents' physical, mental, emotional and social functioning. A wide variety of recreational opportunities are provided daily, including evenings, weekends and holidays. Community outings to parks, restaurants, shops and shows are an important part of each month's calendar of activities. Other programs include outside gardens, church services, crafts, cards, games, cooking, social hours, picnics, bingo, bowling, Nintendo Wii and many other special events.
Private Rooms
Big Meadows offers many unique services. One of the most unique is private rooms for each resident, regardless of payment source. This allows each individual to create their own living space, filled with personal items and memoirs. Families are encouraged to decorate and arrange the room in any way they like to truly make it personalized and comfortable. A Memory Box is provided to display mementos outside their room, allowing others to learn more about each resident. A two-room suite may be available for those couples wanting to continue to share their space.
Respite Care
Big Meadows provides respite services for short-term stays to accommodate vacations or just to give caregivers a well-deserved break. Big Meadows' Respite Services provides reassurance and the peace of mind that comes from knowing that a loved one is well cared for.
Hospice Care
Big Meadows contracts with several area Hospice agencies, who provide additional supportive care and medical interventions. Big Meadows' Hospice Suites provide privacy for family members.
Social Services
The Social Services department assists residents and families with their adjustment to life at Big Meadows. Social Services is the contact for families and loved ones, and as an advocate for residents to assure that needs and concerns are addressed. Social Services also conduct psychosocial group therapy and support groups, and makes referrals to other support services as appropriate. our Social Services staff can also provide assistance with financial issues.
